Use the form below to sign up for the Teller County HDSA Golf Tournament.

Entry price is $125 per player.

If you are only signing up for yourself and are already part of a team, please put the Team Lead Name in the block below.

If you are signing up an entire team, please put all four team member names (including yourself if you are playing) in the Team Members box.

If you sign up to be a $1,000 sponsor, you also receive entry for four team members. Please put all four team member names (including yourself if you are playing) in the Team Members box.

Once you have signed up with the form below, you will be given directions on how to arrange payment.
